This position is responsible for facilitating patient transfer and/or transportation to next site of care settings such as inpatient psychiatric units, Responsible for facilitating patient transfer and/or transportation to next site of care settings i.

based on referral from Social Worker or Psychiatrist.
Coordinates the referral and transfer of patients in an efficient and cost-effective manner.
Provides quality patient care considering age specific, developmental and cultural needs.
Demonstrates unit/area designated competencies.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIESCompletes screening of patients' medical records and maintains accurate record of status of activities related to discharge plan.
Facilitates referrals as directed by Social Worker via phone and web-based application (Epic), arranging transfer and/or transportation of patients to inpatient psychiatric units.
Communicates with inpatient psychiatric units to facilitate appropriate patient placement taking into consideration:

patient clinical condition and needsc.
patient insurance coverageDetermines need for and obtains authorization from the patient's insurer for inpatient psychiatric admission and ambulance transportation.
Communicates with insurance companies via phone or on-line systems to complete prior authorizations for services.
Maintains knowledge of insurance authorization requirements.
May provide office support by placing, receiving, and routing telephone calls and messages.
Screens and prioritizes calls.
Performs individual and group paging as requested.
Provides support to team members for day-to-day operational issues such as distributing work among team members, covering for each other as needed in an emergency etc.

Provides patient and family information and education regarding medical and mental health placement issues relevant to the setting.
Acts as liaison between inpatient and Emergency Department settings to ensure continuity of care throughout each site of care, and to ensure communication about and adherence to the established plan of care for each patient.
Contacts inpatient psychiatric units and/or uses the open bed platform to obtain bed availability informationDevelops strong working relationships with various community agencies and outside hospital systems in order to promote expedient transfers.
Develops, reviews, and modifies referral tools and systems to enhance continuity of care planning processes.
Recommends appropriate changes in departmental procedures to improve productivity, efficiency, and cost-effectiveness.
Independently organizes and sets priorities.
Maintains good rapport and cooperative relationships.
Approaches conflict in a constructive manner.
Helps to identify problems, offers solutions, and participates in their resolution.
Maintains the confidentiality of information acquired pertaining to patients, physicians, employees, and visitors to Trinity Health.
Discusses patient and hospital information only among appropriate personnel in appropriately private places.
Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
